OSF HealthCare Deploys AI to Stop Overpayments
OSF HealthCare has deployed SpendRule, an AI-powered contract intelligence system designed to validate service invoices before payment. The system aims to prevent overpayments and ensure compliance, demonstrating a practical application of AI for operational and financial controls within a healthcare system.
- The system targets "purchased services," which account for nearly half of a health system's non-labor spending and represent a $323 billion national market. Industry estimates suggest that over $32 billion is lost annually to preventable overpayments in this category due to complex and rarely reviewed contracts. - SpendRule's technology digitizes contract terms to enable a "4-Way Match," adding a layer of validation to the traditional 3-way match of purchase orders, receipts, and invoices. This allows the system to automatically flag discrepancies between an invoice and the contract's specific terms, such as volume discounts, before payment is issued. - Dave Fergus, Chief Supply Chain Officer for OSF HealthCare, noted that prior to this system, it was unrealistic to manually ensure every invoice line matched the terms of their thousands of multi-page contracts. The platform is designed to integrate into existing Enterprise Resource Planning (ERP) and accounts payable systems without requiring a full replacement. - This AI implementation is part of a broader strategic focus on cost savings at OSF HealthCare. The health system has a goal to save $200 million over five years and has already achieved $56 million in savings in the last two years, partly through automating invoice and purchase order approvals. - OSF HealthCare is not the only early adopter of this platform; other health systems deploying the technology include Kettering Health, MemorialCare, and MUSC Health. SpendRule, the startup behind the platform, has raised $2 million in seed funding from investors including Abundant Venture Partners and the MemorialCare Innovation Fund. - The implementation aligns with OSF HealthCare's public commitment to the responsible use of AI. In December 2023, the organization was one of 39 healthcare providers and payers that signed the White House's "Healthcare AI Commitments" to ensure AI is used safely and effectively. - The move comes as OSF's new CFO, Kirsten Largent, who took the role in November 2024, aims to improve the system's operating margin to 2% in fiscal year 2025, up from 0.3% in fiscal year 2024 and a negative 3% in 2022.
