GLP‑1s: access grows, cautions rise

GLP‑1 weight‑loss drugs are becoming easier to access — the FDA has approved oral semaglutide as the first GLP‑1 pill for weight loss, and Eli Lilly plans to roll out its oral agent orforglipron in 40 countries including India. (AJMC reports the oral semaglutide approval and News9 says Lilly will launch orforglipron in multiple markets.) (ajmc.com) (news9live.com) At the same time, physicians in India are warning that unsupervised GLP‑1 use can cause muscle loss, nutritional deficiencies, and long‑term metabolic problems. Weight-loss drugs that mimic a gut hormone are moving from injections to pills, and doctors in India are warning that easier access is colliding with unsafe self-use. (ajmc.com) (theweek.in) The United States Food and Drug Administration approved Novo Nordisk’s oral semaglutide tablet, sold as Wegovy, on December 22, 2025, making it the first oral glucagon-like peptide-1 drug cleared for chronic weight management. Novo said it planned a United States launch in early January 2026. (ajmc.com) (accessdata.fda.gov) In the Phase 3 OASIS 4 trial cited in that approval, 307 adults without diabetes took oral semaglutide or placebo for 64 weeks, and the semaglutide group posted a mean weight loss of 13.6 percent versus 2.2 percent with placebo. (ajmc.com) Eli Lilly is pushing a rival pill, orforglipron, across more than 40 countries, including India, while it expands beyond the United States market. Lilly said on February 26, 2026 that it had already submitted the drug to regulators in more than 40 countries. (investor.lilly.com) (moneycontrol.com) Lilly’s pitch is partly about convenience. In its February 2026 head-to-head diabetes trial, the company said orforglipron is a small-molecule oral glucagon-like peptide-1 drug that does not require food or water restrictions, unlike oral semaglutide. (investor.lilly.com) These medicines work by acting like a natural hormone that lowers appetite, slows stomach emptying, and helps the body release insulin when blood sugar rises. Indian health guidance says that same mechanism has made them popular for both type 2 diabetes and obesity treatment. (firstpost.com) (cnbctv18.com) India’s regulator tightened oversight on April 1, 2026 after what it described as growing unchecked availability through online platforms, retail pharmacies, and wellness clinics. CNBC-TV18 reported that inspectors checked 49 entities, including warehouses, wholesalers, retailers, and weight-loss clinics. (cnbctv18.com) Government guidance in India says these drugs are not over-the-counter products and should be prescribed only by endocrinologists, internal medicine specialists, and cardiologists. Authorities warned that violations could bring licence cancellations, financial penalties, and legal action. (firstpost.com) Doctors quoted by The Week said unsupervised use can strip muscle along with fat, leave patients short on protein and micronutrients, and create longer-term metabolic problems when people stop treatment without medical follow-up. The same report said many users start the drugs for cosmetic weight loss rather than diabetes care or obesity treatment. (theweek.in) The label for Wegovy still carries a boxed warning about thyroid C-cell tumors seen in rodents, and Indian guidance also flags nausea, diarrhea, constipation, pancreatitis, and medullary thyroid cancer as risks that require supervision. The new pill era is making these drugs easier to take, not simpler to use safely. (accessdata.fda.gov) (firstpost.com)
