US Hospital Cuts Ignite Political Backlash

A sweeping healthcare reform bill has triggered a wave of cost-cutting measures in U.S. hospitals, particularly in rural areas. Democrats are now seizing on the resulting layoffs and service reductions as a central theme for their midterm campaigns, blaming the legislation for threatening access to care.

The legislation at the center of the debate is the "One Big Beautiful Bill Act" (OBBBA), signed into law on July 4, 2025. The Congressional Budget Office projects the act will slash federal spending on Medicaid by more than $900 billion over the next decade. OBBBA introduces new work requirements for Medicaid recipients, more frequent eligibility checks, and restricts states' ability to use provider taxes for program funding. The law also allowed the enhanced premium tax credits for the Affordable Care Act (ACA) marketplaces to expire at the end of 2025, leading to higher premium costs for millions. The cuts land on a system already in crisis. Even before the new law, 146 rural hospitals in the U.S. had closed or stopped providing inpatient services between 2005 and 2023. As of late 2025, an additional 756 rural hospitals were considered at risk of closure, with 323 of those facing an immediate threat. The new law is expected to increase the number of uninsured patients, shifting a greater burden of uncompensated care onto hospitals that were already financially strained. This reversal of ACA-era gains in profitability and lower uncompensated care is a primary driver of the new cuts. Heading into the 2026 midterm elections, healthcare costs have become a top economic anxiety for American voters. A recent KFF poll found that two-thirds of adults are worried about being able to afford healthcare for their families. That same polling shows voters, including independents, trust Democrats more than Republicans on handling healthcare costs. With control of Congress at stake, the hospital cuts have provided a powerful narrative for Democratic campaigns, who are highlighting the immediate impact of the legislation on community access to medical care.
