Zepbound Bests Wegovy in Weight-Loss Study

A new head-to-head study showed Zepbound outperformed Wegovy for weight loss in people with obesity. The result sets a new clinical efficacy standard in the massive weight-loss drug market. This shift could impact everything from payer policies and lab testing volumes to public health trends.

Eli Lilly's Zepbound (tirzepatide) demonstrated superior weight loss to Novo Nordisk's Wegovy (semaglutide) in the SURMOUNT-5 head-to-head trial. After 72 weeks, Zepbound users lost an average of 20.2% of their body weight, compared to 13.7% for those on Wegovy. This translated to an average weight loss of 50.3 pounds for Zepbound patients versus 33.1 pounds for Wegovy patients. The clinical outperformance stems from Zepbound's dual-agonist mechanism. While Wegovy mimics a single gut hormone (GLP-1) to regulate appetite, Zepbound targets both the GLP-1 and GIP hormones. This dual action is believed to provide a more comprehensive metabolic effect, leading to greater efficacy. The competitive landscape extends beyond pure weight loss. Wegovy is approved to reduce cardiovascular risk in certain patients, a key differentiator. Meanwhile, Zepbound recently gained FDA approval for treating moderate to severe obstructive sleep apnea in adults with obesity. The market for these anti-obesity medications is forecast to be substantial, with some estimates projecting it could reach $133 billion by 2031. North America currently represents the largest market for these drugs. This clinical result could shift market share between the two pharmaceutical giants. Despite clinical efficacy, patient access remains a significant hurdle. Without insurance, the monthly cost for Zepbound can range from $1,200 to $1,500, while Wegovy can cost between $1,300 and $1,600. Coverage varies widely, with insurers often requiring prior authorization before approving the medications.
