- The enhanced premium tax credits from the American Rescue Plan Act, later extended by the Inflation Reduction Act, expired on December 31, 2025, after Congress failed to pass legislation to continue them. - The Urban Institute, a nonprofit research organization, estimates that 125,000 Native Americans are at risk of becoming uninsured in 2026 due to the rising costs of ACA plans without the enhanced subsidies. - The Indian Health Service (IHS), which is obligated by treaty to provide healthcare to Native Americans, is chronically underfunded. To supplement its budget, the IHS relies on third-party billing, which generated $1.8 billion in 2023 from sources including ACA marketplace plans. - For Native Americans with an income up to 300% of the federal poverty level, the Affordable Care Act includes provisions for zero cost-sharing on marketplace plans, meaning no deductibles, copayments, or coinsurance. - Without the enhanced subsidies, the Tuba City Regional Health Care Corp. in the Navajo Nation anticipates its monthly costs for patient coverage to increase by approximately 170% to nearly $38,000. - Many tribal nations have established their own health insurance programs that help enroll their citizens in ACA marketplace plans to provide access to specialists and services not readily available through the Indian Health Service. - Before the subsidies expired, about 318,000 Native Americans were enrolled in ACA plans. Following the expiration, overall enrollment in ACA plans dropped by more than 1 million people nationwide.
