Kaiser Therapists to Strike Over AI Concerns

Some 2,400 mental health therapists at Kaiser Permanente are set to strike on March 18, with unions citing concerns over the HMO's increasing use of AI to replace human therapists. This specific labor action centers on Kaiser's increasing use of AI, which therapists fear will lead to layoffs and a lower quality of patient care. The union has filed an Unfair Labor Practice charge against Kaiser for unilaterally changing its patient triage system to rely more on non-clinical staff and AI. This isn't the first time NUHW has clashed with the healthcare giant; a 10-week strike in 2022 and a nearly seven-month strike that began in late 2024 centered on improving patient access and therapist time for patient-related duties. Therapists argue that Kaiser's new system, which uses yes-or-no prompts and telephone operators alongside AI, can delay urgent care and result in improper patient assignments. The union contends that mental healthcare relies on a human connection that technology cannot replace. Kaiser Permanente maintains that AI is a tool to assist, not replace, healthcare professionals, helping with tasks like diagnostics and administrative work to give clinicians more time with patients. The company has stated its commitment to the responsible use of AI and is involved in national initiatives to establish ethical guidelines for AI in healthcare. However, the union remains skeptical, pointing to contract proposals that they say would give Kaiser the authority to lay off therapists and further outsource mental health services. The backdrop to this dispute includes significant regulatory scrutiny of Kaiser's mental health services. Since 2023, Kaiser has faced over $230 million in penalties for mental health violations in California. A state investigation following the 2022 strike found that Kaiser had canceled over 111,000 therapy appointments.
