GLP-1s Studied for Addiction

Researchers are now investigating if GLP-1 drugs like Ozempic could help reduce addictive behaviors. Early findings suggest the medications might fight addiction, potentially expanding their use far beyond diabetes and obesity into new therapeutic areas.

GLP-1 agonists appear to work on the brain's mesolimbic system, also known as the dopamine reward pathway. This is the same region that reinforces cravings for addictive substances like nicotine, alcohol, cocaine, and opioids. By influencing this pathway, the medications may blunt the "craving that pulls people toward whatever they're addicted to." A large-scale study of over 600,000 U.S. veterans with type 2 diabetes provided significant data on this potential application. The research, led by Dr. Ziyad Al-Aly from Washington University School of Medicine, compared patients on GLP-1 drugs to those on other diabetes medications. The results showed a marked decrease in new substance use disorders among those taking GLP-1s. The risk of developing an addiction was reduced by 18% for alcohol, 20% for cocaine and nicotine, and 25% for opioids. For patients who already had a substance use disorder, the impact was even more significant. Taking GLP-1s was linked to a 39% lower risk of overdose and a 50% reduction in drug-related deaths over a three-year period. The effect isn't limited to illicit drugs and alcohol. Preclinical data shows GLP-1 agonists reduce nicotine self-administration in rodents, and early human trials suggest they may help lower the number of cigarettes smoked per day. While these observational findings are strong, researchers emphasize the need for more clinical trials to confirm the drugs' efficacy and safety specifically for addiction treatment. Currently, GLP-1s are not FDA-approved for treating addiction, which can create insurance coverage barriers for this off-label use.
