GLP‑1 muscle loss flagged

- A review found muscle-related tissue made up about 35% of total weight lost in incretin (GLP‑1) treatment groups. (ndtv.com) - The review reported muscle loss exceeded safe benchmarks in roughly two‑thirds of reviewed cases. (ndtv.com) - Indian outlets also reported seizures of counterfeit Mounjaro injections valued in the tens of lakhs, underscoring safety and supply risks. ( )

Glucagon-like peptide-1 drugs help people lose weight, but a growing stack of studies says some of that loss comes from muscle-related tissue, not just fat. (nature.com) These medicines work by mimicking gut hormones that reduce appetite and slow stomach emptying; tirzepatide also activates glucose-dependent insulinotropic polypeptide, or GIP, alongside glucagon-like peptide-1. In a body-composition substudy of the STEP 1 trial, people on semaglutide lost 15.0% of body weight over 68 weeks, including a 9.7% drop in total lean body mass. (academic.oup.com) A 2024 review in *Diabetes, Obesity and Metabolism* said lean-mass loss across glucagon-like peptide-1-based therapies typically ranged from 0% to 40% of total weight lost. A 2025 systematic review said studies of tirzepatide found fat mass fell by about three times as much as lean mass, pointing to a similar pattern rather than a uniform outcome. (dom-pubs.onlinelibrary.wiley.com, pmc.ncbi.nlm.nih.gov) Researchers are focusing on muscle because lean tissue helps with strength, balance, daily function and glucose disposal. A July 2025 comment in *Nature Reviews Endocrinology* said rapid weight loss with glucagon-like peptide-1 receptor agonists can come at the cost of skeletal muscle and called for treatment plans that actively preserve it. (nature.com) The signal is not simple. A 2025 prospective study of 115 patients treated with semaglutide 2.4 mg reported significant weight and fat-mass loss with muscle function improvement and said lean mass was preserved, showing that results can vary by population, measurement method and follow-up. (pmc.ncbi.nlm.nih.gov) That is why clinicians increasingly pair these drugs with protein intake, resistance training and slower dose escalation instead of treating the injection as a stand-alone fix. Recent reviews and commentaries describe those steps as the main strategies under study to limit avoidable muscle loss during pharmacologic weight reduction. (dom-pubs.onlinelibrary.wiley.com, pubmed.ncbi.nlm.nih.gov) The safety picture is also widening beyond side effects listed on the label. The current U.S. prescribing information for Mounjaro, Eli Lilly’s tirzepatide brand for type 2 diabetes, includes a boxed warning about thyroid C-cell tumors seen in rats and says the pen should never be shared between patients. (accessdata.fda.gov) In India, demand has also drawn counterfeit supply. Gurgaon authorities said on April 20, 2026, that they seized suspected fake Mounjaro KwikPens worth more than Rs 70 lakh, with packaging irregularities, labeling mismatches and storage violations flagged during the raid. (economictimes.indiatimes.com, yahoo.com) So the current medical debate is shifting from how much weight these drugs can remove to what kind of tissue patients are losing along the way. The next phase of evidence is likely to focus less on the scale alone and more on body composition, strength and verified supply. (nature.com, dom-pubs.onlinelibrary.wiley.com)
