Global FHIR Standards Development Advances
A recent meeting on Australia's Patient Summary FHIR Implementation Guide focused on standardizing clinical data to improve portability between care settings. The session emphasized mapping local clinical terms to international vocabularies like SNOMED CT and LOINC to achieve semantic interoperability.
- The 21st Century Cures Act, overseen by the Office of the National Coordinator for Health IT (ONC), mandates that healthcare providers give patients secure access to their electronic health information (EHI) at no cost. This is often accomplished through standardized application programming interfaces (APIs), which has led to an increase in hospitals enabling patient access to their data via FHIR-based apps. - While mapping local terms to SNOMED CT and LOINC is crucial for interoperability, challenges exist because LOINC is often more granular and precise in defining laboratory tests than SNOMED CT. This can result in one SNOMED CT concept mapping to several LOINC codes, and a significant portion of LOINC codes may not have a direct equivalent in SNOMED CT. - To transition into nursing informatics, an RN with a BSN can pursue the Informatics Nursing Certification (NI-BC) from the American Nurses Credentialing Center (ANCC). Eligibility typically requires two years of full-time practice as an RN, 30 hours of continuing education in informatics, and either 2,000 hours of informatics nursing practice in the last three years or 1,000 hours plus 12 credits in a graduate-level informatics program. - ICU experience is highly valuable for an informatics role as it provides a deep understanding of clinical workflows, data-driven decision-making in high-pressure situations, and the usability challenges of EHRs for frontline staff. This background is critical for designing and implementing technology that genuinely supports patient care rather than hindering it. - Common complaints from nurses about EHRs include physician-centric design, excessive clicks and redundant data entry, and a lack of mobile-friendly interfaces. Many nurses report that EHRs can inhibit efficient, quality patient care and are a significant contributor to burnout. - Epic EHR optimization projects have shown significant success in improving nursing workflows. One initiative at UCHealth reduced the time nurses spent on documentation by 18 minutes per 12-hour shift, saving over 64,800 hours annually by redesigning flowsheets and removing unnecessary documentation fields. - AI-driven clinical decision support is increasingly used in critical care to predict patient deterioration, identify early signs of conditions like sepsis, and optimize treatment pathways. These tools can analyze vast amounts of real-time data from the EHR and patient monitors, helping to reduce cognitive overload for ICU clinicians. - The ONC's Cures Act Final Rule also includes provisions to prevent "information blocking," which is any practice likely to interfere with the access, exchange, or use of electronic health information. This is intended to promote competition and innovation in healthcare technology.
