New Healthcare Law Sparks Hospital Cuts

American hospitals are making significant operational cuts, including staff reductions and service rollbacks, after the passage of the new "big beautiful bill" healthcare reform. The cuts are becoming a major political issue, with Democrats using them as ammunition for the upcoming midterm elections, arguing the law is hurting access to care.

The "big beautiful bill," officially known as the One Big Beautiful Bill Act (OBBBA), was signed into law on July 4, 2025. It enacts over $1 trillion in healthcare spending cuts over the next decade, primarily from Medicaid. The legislation also introduces new work requirements for some Medicaid recipients and alters rules for those receiving premium tax credits through the Affordable Care Act marketplaces. These funding changes are having a significant impact on hospitals, particularly those in rural areas. Over 300 rural hospitals are considered at risk of closure due to their higher percentage of Medicaid patients. The law includes a $50 billion "Rural Health Transformation Fund" to mitigate these effects, but this is not expected to cover the full extent of the revenue loss from the Medicaid cuts. Hospitals and health systems across the country have already begun to react to the new financial pressures. For example, Vanderbilt University Medical Center has announced up to 650 layoffs, UC San Diego Health has eliminated over 200 positions, and Seattle Children's Hospital plans to lay off more than 150 staffers. In some areas, entire hospital wards, such as obstetric services, have been shut down. The Congressional Budget Office projects that the changes in the law will lead to an increase of roughly 10 million uninsured people by 2034. Hospital administrators anticipate that many of these individuals will shift from being covered by Medicaid to receiving uncompensated care, further straining hospital finances. This has led to predictions of longer emergency department wait times and patients delaying necessary medical treatment.
