Data Ranks Most Competitive Medical Specialties

Recent match data shows dermatology, plastic surgery, and orthopedic surgery remain the most competitive medical specialties to match into. The rankings are driven by high applicant-to-position ratios, with radiology and other interventional specialties also seeing rising competition.

The intense competition for top residency spots is largely driven by compensation and lifestyle. Orthopedic surgery, for example, boasts the highest average annual salary at $564,000, with plastic surgery following at $544,000 and radiology at $526,000. Dermatology, another top-tier specialty, offers an average salary of $454,000 combined with a more predictable, office-based schedule with rare after-hours emergencies. Historically, a high USMLE Step 1 score was a critical filter for these elite specialties, with matched applicants in plastic surgery and dermatology averaging scores around 249 and 248, respectively. Since the exam became pass/fail in 2022, the emphasis has shifted dramatically to the USMLE Step 2 CK score. For the most competitive fields, a top-tier Step 2 score is now considered essential. Matched applicants in dermatology and orthopedic surgery have average Step 2 scores of 257. Aspiring plastic surgeons and otolaryngologists (ENT) also need to aim high, with average matched scores around 256. Beyond test scores, a deep research background is becoming a key differentiator. The average number of research publications and presentations for matched applicants in the most competitive specialties has seen a significant increase. For instance, successful applicants for neurosurgery and dermatology now average over 27 research items. The path to matching in these fields is exceptionally narrow. Dermatology has a match rate of just 70.5% for applicants. This is a result of high demand for a limited number of training positions; annually, more than 600 applicants vie for about 500 dermatology residency spots. While surgical specialties like neurosurgery and plastic surgery have long been famously competitive, other fields are seeing a rise in interest. Interventional radiology, a specialty focused on minimally invasive, image-guided procedures, is increasingly competitive, with matched applicants having an average Step 2 score of 255.
