Sanofi Touts Long-Term Drug Data

Blueprint Medicines, a Sanofi company, announced new four-year data from its PIONEER study for the drug AYVAKIT. The company says the long-term results show a sustained benefit and safety profile for patients with indolent systemic mastocytosis, a rare blood disorder.

Indolent systemic mastocytosis (ISM) is a chronic hematologic disorder characterized by the abnormal accumulation of mast cells in various tissues, including the skin, bone marrow, and gastrointestinal tract. This buildup can lead to a range of symptoms, from skin lesions and flushing to gastrointestinal distress and the risk of severe allergic reactions. The vast majority of ISM cases, approximately 95%, are driven by a specific genetic mutation known as KIT D816V. This mutation causes the KIT protein, a type of enzyme called a tyrosine kinase, to become constantly active, leading to the uncontrolled growth and survival of mast cells. AYVAKIT (avapritinib) is a potent and selective tyrosine kinase inhibitor designed to target this specific KIT D816V mutation. By blocking the activity of this mutated enzyme, the drug interrupts the signaling pathways that fuel the overproduction of mast cells. Before its approval for ISM in May 2023, avapritinib was first approved by the U.S. Food and Drug Administration (FDA) in January 2020 for treating gastrointestinal stromal tumors (GIST) with a specific mutation. It also received approval in June 2021 for advanced systemic mastocytosis, a more aggressive form of the disease. The PIONEER trial was a Phase 2, randomized, double-blind, and placebo-controlled study, making it the largest and most rigorous trial of its kind for indolent systemic mastocytosis. Patients in the trial received either AYVAKIT or a placebo in addition to their existing best supportive care medications, which can include antihistamines and leukotriene inhibitors. Prior to targeted therapies like AYVAKIT, treatment for ISM primarily focused on managing symptoms. Standard care involved H1 and H2 antihistamines to address skin and gastrointestinal issues, mast cell stabilizers, and strict avoidance of known symptom triggers.
